The QQE06-40, from Mullard's owners Philips, appears to be an earlier form of the european pattern double tetrode. It is taller and there are no obvious sign of the internal neutralising capacitors. The QQV06-40A from Mullard was a standard valve for VHF transmitters of medium power.
Mullard do not give a value for the 'E' in their naming convention for transmitting valves but we have another valve with the 'E' and that is marked Mullard.
The wide glass tube envelope is 43 mm in diameter and, excluding the B7A base pins, is 87 mm tall.
