The CV4055 is a video output valve. It is a beam tetrode with the beam plates being formed from four vertical support rods. The cathode is flat and the grids shaped to pass across the cathode at a constant distance from it.
The 6CH6 is the commercial valve and that entry has additional information.
The anode shape reveals the grid structure. The four vertical rods form the beam plates.
The thin glass tube envelope is 20 mmin diameter, and excluding the B9A base pins, is 57 mm tall.
