This 12A6 metal valve was made by Ken-Rad in America. In addition to the commercial Type designation it also carries the USA service code of VT-134.
The 12A6 was designed for use in the single end audio output stages of radio receivers.
The maker's name.
The thin metal tube envelope is 25 mm in diameter and, excluding the IO base pins, is 67 mm tall.
